Although you are welcome to visit 24 hours a day, the safety and health of our resident is our number one priority. Therefore if visiting out of the 8am to 8pm hours we suggest you call ahead to ensure access as we do lock our doors while our residents are asleep to ensure they feel secure and comfortable in their community and home.
The Living Centre does have a laundry service, so if you wish us to take care of the laundry for the resident please let us know upon admission. If the resident wishes family or a caregiver to handle laundering their items please let us know upon admission so staff can have a sign placed in the room to notify housekeeping of this choice. We do recommend a laundry hamper for the resident’s clothes and that family or caregiver pick up the clothing twice per week to keep up with health and sanitary practices. In accordance with guidelines personal clothing is washed in an industrial washing machine at a temperature of 180 degrees. We ask that the residents clothing is appropriate for washing and drying in these hot temperatures. Avoid clothing that requires special care, or is subject to damage and fading. All clothing must be labeled with resident name, please do not label with initials or room number to prevent confusion. It is best to mark in large, clear letters on the inside neckline or waistband. All items must be marked including socks, shoes, and other personal items.
We encourage children to visit, however, we ask you to alert the charge nurse to anyone visiting the facility under the age 12.
We would love for your family pet to visit! Please check in at the front office so we can make sure they are current on all vaccinations and are going to be kept on a leash.
Mail will be delivered to residents on a daily basis except for weekends and holidays.

If the resident chooses we provide cable for $25/month, however the residents bring their own televisions from home. We do have a community television in the common areas for residents if they choose not to have them in their rooms.
The Living Centre has a vibrant atmosphere with dynamic, individualized activities that match the capabilities and needs of residents and their guests. We also have active resident and family councils and volunteer programs. Scheduled activities include: music, fitness activities, religious meetings, outside entertainment, games, gardening, field trips/outings and volunteer involvement. If you would like to arrange something special or have suggestions and ideas for individual or group activities, please inform our activities director. We welcome and encourage involvement from family and friends.
A Certified Nursing Assistant (CNA) is a health professional that works under the supervision of Registered Nurses (RN's) and Licensed Practical Nurses (LPN's) to provide hands on resident care, specifically with activities of daily living (ADL’s), vital signs, ambulation, and positioning.
An LPN provides basic bedside nursing care to residents under the direction of a physician or registered nurse. Duties within the scope of practice for an LPN typically include, but are not limited to, provision of basic hygienic and nursing care; measurement of vital signs; basic client assessment; documentation; performance of prescribed medical treatments; administration of prescribed medications; and, performance of non-medicated intravenous therapy and blood withdrawal.
An RN's duties may include everything a CNA and LPN carry out, in addition to administering an IV, assessment of residents, skin care, and processing doctors' orders. Furthermore, RN's typically take on management positions such as the Director of Nursing, who is responsible for all nursing duties in the building.
We strive to create social programs and individualized activities to match the capabilities and needs of each resident. This also has a powerful impact in the healing process of those residents involved in some type of rehabilitation. Specific activities include: active resident councils, musical performances, current events update, reading the newspaper, singing exercises, fitness activities, stretching exercises, religious meetings, outside entertainment, games, trivia, history, and volunteer involvement. We also strongly encourage families to participate with the residents in various activities.
A monthly calendar is posted to keep residents informed of upcoming events such as birthday parties, movies, musical programs, outings, candlelight dinners, ice cream socials and other special events. We always welcome friends and families to visit and attend our special events.
We recommend bringing about five changes of clothing as well as a pair of comfortable rubber-soled shoes. We ask that the clothing comes washed, clearly labeled with resident's name in permanent ink. We also advise that residents mark other personal items for identification. Although we strive to maintain privacy and security of items we do have a variety of residents at he Living Centre so we ask that residents do not keep valuable jewelry or large amounts of cash in their rooms.
We can reserve different areas in the facility for special events, like birthdays, holidays, and other meals, with some notice.
Since we want to keep our Residents safe and healthy we will allow a leave of absence from our facility but only with a physician’s authorization. Our nursing staff can help arrange that as well as sign the resident out of the facility through the LOA Book (leave of absence) when leaving and returning.
There is ample parking in the parking lot with wheelchair access. Please observe all posted city parking signs. Avoid parking in spaces specifically reserved for the handicapped, employees and physicians.
A phone line does cost $20/month for the resident, and we can provide a standard phone set. If the resident has a phone they prefer however, we can set that up as well. They will each have a private extension they can be reached at and we will give that to the family upon admission.
WiFi Internet is provided for residents and their guests, please see the front office for the password.
If residents have a Medicare card that reads "Hospital Insurance.”, and have been admitted to the facility with a physician order within 30 days of a three consecutive night hospital stay, then Medicare will cover skilled nursing. The first 20 days of care, in general, Medicare will pay 100%. Once skilled care has reached day 21-100 there is a daily co-insurance co-pay that is required. After the 100 days Medicare usually will not cover skilled nursing fees.
Our primary concern is that your loved one feels comfortable here at The Living Centre and that their needs are being met. You should feel secure and at ease with our staff, particularly those providing direct care. If you have any concerns or praise for our staff, please feel free to contact our Administration and/or Social Services department. They will be able to address any apprehensions you may have regarding any particular staff members.
An Ombudsman is an advocate for residents, they are specifically there to be a voice for the residents and their concerns. To be clear they are NOT employed by the facility, which means they will not tell a facility what to do but they will be around periodically monitoring how we care for resident concerns.

Our social services department assists residents and their families during transitional periods. They also provide essential information, manage requests and concerns, help in care and discharge planning for each resident. They can arrange professional services for: Dental, Vision, Podiatry, Audiology, Power of Attorney, Dementia assessment, Psychosocial and Psychiatric Evaluation, and Individual therapy.
Although we do not have a formal policy stating visitation guidelines, we suggest using the common areas of the facility to meet with your loved one. If it is not possible to visit in one of the common areas, we suggest a limit of one to three people in a resident room at one time.
In general physicians to see the residents on admission and then will schedule a visit once a month for the first three months. Our nursing staff does stay in contact with the resident’s primary physician so that if there is any concerns the physician is aware. If your insurance company requires more visits from a physician check in with our nursing staff to schedule transportation.
For the safety and welfare of all residents and staff, The Living Centre is a non-smoking facility.
We do allow food to be kept in a resident’s room in properly sealed containers, however please make sure the food you are bringing is in line with any special diets or health provisions. If you are not sure if your loved one has any health, or diet provisions please ask guidance from our nursing staff.
There is a beautician available by appointment on Tuesdays and Thursdays from 9am-3am. Please speak with our business office for appointments and prices.
Our rehabilitation team can inform you of the rehabilitation program designed for your loved one.
During admission our Dietary Manager or Dietitian will discuss special needs and food preferences. The meals are prepared by our kitchen staff and special attention is payed to serve food that is appetizing and flavorful. The menus follow all medical diets and specialization prescribed by the dietitian and physicians.
There are alternatives to each daily menu so that you have choices. If you wish to not have the main item on the menu for the day please let your CNA know so that they can pass the information along to the Dietary Manager and they will try and accommodate your needs. Our Dietitian will monitor all nutritional data and alert you to any changing needs
The Living Centre’s Business office handles all financial and billing matters. If you have any questions please talk with our receptionist about making an appointment with our Business Office Manager.
We do have you meet with an Admissions Coordinator during your admission process. This coordinator will guide you through all of the paperwork that is required as well as the rules and regulations that are involved with your specific pay source.
