Skilled Nurses are staffed 24/7 to provide quality care when you need it. Our Nurses are not only caring and genuine, they individualize their care to each resident.

Our comprehensive approach to nursing, rehabilitation, and support services includes post-hospital, post surgical, and restorative care in comfortable private and semi-private accommodations with 24-hour licensed nurses.

Rehabilitation Services

Our rehabilitation team is committed to results, and improving our resident’s quality of life in and out of our facility. We strive to increase resident’s independence through individualized programs and goals. We highly encourage family to participate in, not only the therapy program, but other training and education provided. This will help our resident achieve their goals.

We have an experienced Physical Therapy team that works with individualized goals to achieve the resident’s highest level of independence possible. Our physical therapists will identify areas of physical impairment and pain, and create specific plans for rehabilitation that will help prevention future injuries.

These are some areas that our Physical Therapists might focus on:

  • Increase strength and mobility
  • Increase motor control
  • Improve coordination, balance and the ability to walk
  • Improve overall strength conditioning, range of motion and endurance
  • Pain management
  • Teaching proper body mechanics
  • Promoting safety awareness

If a resident’s independence has been compromised due to illness, injury or aging, we have qualified occupational therapists who can put together active treatment plans to gain that independence back. They work to create a task-oriented plan that will help residents adapt to their social and physical community by mastering those tasks.

Areas of treatment may include:

  • Activities of Daily Living: eating, bathing, toileting, homemaking, cooking, etc.
  • Prevention: Assessments of homes, suggested home modifications and adaptive equipment.
  • Increasing safety awareness

Our speech therapists will assess and help restore functional abilities in the common areas affected by stroke, brain injury, dementia and other neurological disorders. They will provide treatment plans and interventions in the areas of communication, swallowing and cognition.

Areas of focus can include:

  • Improving speech and voice clarity
  • Increasing comprehension and verbal expression
  • Improving chewing and swallowing ability
  • Maximizing cognitive language skills: memory, problem solving, and abstract reasoning
  • Resident and family/caregiver education and training for managing ongoing difficulties with communication, cognition, and swallowing

Other Services

Sub-acute care is for residents who are medically fragile and require special services, such as inhalation therapy, tracheotomy care, intravenous tube feeding, and complex wound management care.

We are pleased to provide a sub-acute care program for residents recently released from the hospital. We are able to treat residents, in whatever state they may be in, from recently discharged from a hospital to medically fragile. Our goal is to help our residents in the recuperation process while helping them achieve their maximum capabilities to return home as quickly as possible.

Severe injury often affects the whole person – body, mind and spirit. Our nurses are available 24 hours a day and our therapists, dietitians, case managers are available to fit your schedule. Within our sub-acute care we also offer social services as well as enrichment activities.

Whether it’s Dementia or Alzheimer’s, our staff is able to help determine a positive course of action on a case by case basis. Our staff here at The Living Centre know that when someone is suffering with the progressive stages of memory loss, they will need continued support and assistance. We also understand that each case is uniquely different and deserves the utmost attention and care. We offer the care and attention needed to help with this process in each unique case.

Our personalized care focuses on:

  • Creating appropriate treatment options for each resident
  • Monitoring treatment effectiveness as each resident progresses
  • exploring alternatives as needed
  • Creating individual goals for treatment and tolerance

Dietary Program

Our meals are designed to specifically meet dietary needs and physician orders. Our registered dietitian will assess, and continue to monitor residents to make sure they are receiving top nutrition for their specific goals.

Activites Program

The Living Centre strives to create enriching activities and social programs that will keep residents active and confident. Our friendly and outgoing professional staff reach out to all residents to brighten their day, and stay attentive to the culture and abilities of the residents. It is well known that activities and social interaction has a positive impact on the healing process and mental wellbeing.

Each resident is given a monthly activity calendar, as well as Daily News, that helps keep them apprised of upcoming events as well as current day to day of the community. We welcome friends and family to join their loved ones for activities and events.

Social Services

During those transitional periods The Living Centre offers a social services department than can assist in those confusing and emotional decisions. Our social services department is here for both the resident and family to assist in getting the essential information and managing the process. Our Social Worker will help with care planning and discharge planning for each individual resident.
